How to Refer Your Patient to the NOSC

At the National Obesity Surgery Centre we do all that we can to make everything as simple and uncomplicated as possible, both for doctor and patient! Consequently, we have three easy ways for you to introduce your patient to the Centre:

• Telephone the Centre on 0845 618 7276 during office hours
• E-mail your patient’s brief details on the form below
• In writing by regular mail . Click here for details

At this stage, all we need is the very briefest of details. Following your referral, we shall contact your patient directly and arrange a consultation. Should your patient decide to proceed with the NOSC Programme, we would then require a formal letter of referral from you.

You will then be regularly kept informed of your patient’s progress.
